INTRODUCTION

You are invited to take part in this study because you are a physician in Canada.

Your participation is voluntary. If you decide to participate, you will be asked to complete this survey. If you do decide to take part in this study, you are still free to withdraw at any time and without giving any reasons for your decision.

Please take time to read the following information carefully. If you have any questions, please contact one of the investigators listed at the top of this form.

WHY IS THIS STUDY BEING DONE?

Research on violence experienced by physicians, especially Canadian studies, are lacking. Canadian studies that do exist primarily focus on family physicians. We aim to analyze the prevalence of workplace violence experienced by Canadian physicians across all specialties, and use the information to address barriers in accessing various supports during and after an incident, as well as systemic change around workplace safety.

WHAT DOES THE STUDY INVOLVE?

You are invited to complete a 10-15 minute survey about psychological, physical, and sexual violence encountered in the workplace. Your participation in this survey is voluntary. You may stop the survey at any time without affecting your relationship with the Principal Investigator or the study team. You may refuse to answer any survey questions by leaving them blank.

RISKS AND BENEFITS

There are no known or anticipated risks to you by participating in this research. Our findings could potentially provide information to improve access to supports during and after a violent incident as well as address systemic change around workplace safety.

CONFIDENTIALITY

In Saskatchewan, the Health Information Protection Act (HIPA) defines how the privacy of your personal health information must be maintained. Your confidentiality will be respected. The results of this study may be presented in a scientific meeting or published, but your identity will not be disclosed.

The electronic data will be stored on REDCap for at least 5 years after the result of this study may have been presented at a conference or published. Then, all study participant data will be electronically deleted.

Your responses will remain anonymous. No identifying information, such as name, address, or birthdate, will be collected. Responses will be kept confidential. Only aggregate data will be reported. The results of this research will be used for presentations and/or publications.

When providing response to a question in which you are asked to elaborate, please do not include any personally identifiable information (e.g. names, email address) as researchers cannot act on any instances reported. If you wish to report a personally experienced abuse, please contact your regulatory body.

RIGHT TO WITHDRAW

Your participation is voluntary. If you do decide to take part in this study, you are still free to withdraw by not submitting your survey responses. Once submitted, it will not be possible to remove your response. If you choose not to participate, this will have no effect whatsoever on your involvement with the researchers.
