Submitting Your Application for Operational Approval (OA) 


Please read before you begin: 

Our offices will be closed during the holidays from December 21st, 2024 - January 5th, 2025 inclusive. On January 6th, 2025, we will begin processing the applications submitted during the holiday season in the order received. 
Research studies requiring access to SHA resources or patients (including their personal health information) must obtain two types of approval: Research Ethics Approval and Operational Approval. 
Do not submit your application for Operational Approval until AFTER the research ethics application has been submitted.... 
d.       Click "Survey settings" to the right of the data collection instrument "Operational Approval Form_New and Amended" 
e.       Scroll to the bottom for "Survey Completion Text" to add the following (see highlighted in yellow): 
  
Thank you for applying for Operational Approval for Research. A Coordinator will be in touch within 2 - 4 weeks.
